In the event of divorce or separation, the divorce decree (or legal separation document) will serve as proof of parental authority.<\/p><p>Please make sure to arrive at the scheduled time for your appointment. If you are unable to move around, a relative may complete these procedures on your behalf. For minors, the responsible adult (parent, guardian or authorised adult, where applicable) must be present at discharge and provide a valid ID document.<\/p><p class=\"p1\">If you decide to leave the clinic against medical advice, you will be required to sign a discharge form releasing the clinic from liability.<\/p><p class=\"p1\">At the end of your stay, your personal medication will be returned to you. If necessary, the referring doctor will provide you, or your general practitioner, with an updated prescription.<\/p><p class=\"p1\">Prescriptions are written using the International Nonproprietary Name (INN).
